Type	Form	Description
	Primacare Registration Form PR1	This Form can be used to Register yourself and your family with all of our Primacare Medical & Dental centres throughout the country, one form should be filled per person and sent to your nearest Primacare Medical & Dental Centre.
	Medical Card and GP Visit Application Form	This form can be used to apply for a Medical Card or GP Visit Card
	Health Expenses, Claim for Relief - Form Med1	This form can be used for claiming relief on medical expenses paid by you ( Please find this one attached to this mail )
	Change of Doctor	This form is use when you have a medical card and you wish to change from one doctor to another.
	European Health Insurance Card	This form is used if you have a medical card and you want to apply for a European health Insurance card.
	Eyesight Report Form	This Form is used when you are applying for a driving license and require and eyesight report.
	Maternity Benefit Forms MB 10	This form can be used for claiming maternity Benefit from the social welfare services
